After 47 years of serving our community and welcoming more than 16,000 babies, it is with very heavy hearts that we share that Lifecycle Wellness and Birth Center will be closing its doors in 2026. 

Guidelines for Getting Your Records and Transferring Your Care

We want to make this process as smooth as possible for you. Follow the steps below to request your medical records and, if needed, transfer your care to another provider.

1. Requesting Your Records

You have the right to a copy of your medical records.

To request them:

  • Include:
    • Your full name and date of birth
    • The dates of care you’re requesting
    • The name, address, and fax number of the provider or facility where records should be sent; incomplete details will not be processed.

We’ll process requests within 10 business days. You’ll receive a portal message once your request is complete.

2. Transferring Your Care

  • Choose your new provider and schedule an appointment.
  • If you are currently pregnant, we will be in touch with you, either by phone or at your next visit, to assist you in finding a practice that aligns with your needs.
  • Ask your new provider if they need your records before your first visit. Many do, especially if you’re pregnant or mid-treatment.
  • Submit a release form authorizing us to send your records directly to the new provider.

4. Important Reminders

  • We cannot release your records without a signed authorization form.
  • Records include prenatal and postpartum visit notes, lab results, ultrasounds, and delivery summaries (if applicable).
  • If you received care at the hospital, those records must be requested directly from the hospital.
